Skin tightening is done to revive that loss of tightness by firming the skin. The process addresses what Dr. Schultz calls “the envelope of your skin,” which is its outer two layers, the dermis and epidermis. As your skin gets older it loses elastic tissue and becomes stretchy, also aggravated by good old gravity, so the goal is to “unstretch” it. Dr. Schultz explains further, “Skin tightening is almost like making a messy bed — you make the bed tighter so the sheets and bedspread are no longer loose. When you see people trying to bounce a dime on the bed, that’s really what we’re talking about with skin tightening. It does help with some fine lines, but it’s more about firming.” Skin tightening will give you very good results at the lower lids, jawline, the lower face and neck. Dr. Schultz adds, “You can also get very good results in the area next to the mouth where people get little puffy things I call poofs.”
When skin tightening with IPL, you’ll typically need five to six treatments over three to four weeks with a touch up at six months. “But,” Dr. Schultz advises, “the problem is people like the results so much (because they’re a subtle 30-40% improvement) that they come back earlier for their touch ups and some ask to come in every month!” With Exilis, you’ll receive a deeper form of tightening because the energy (a form of radiofrequency energy) in that medical device is greater and ends up going deeper. You’ll do three to four treatments at one week intervals and once a year you’ll do a touch up.
A regimen of skin tightening is a great complement to botox and fillers. Botox is used to stop muscles from contracting that otherwise cause lines and wrinkles now, but it also prevents them from becoming etched permanently when used regularly. Filler fills back volume that’s been lost in the fat layer and dermis, the second and third layers of skin under the (top layer) epidermis and synergizes with the improvements of skin tightening. Filler replaces some of the lost volume resulting from lost fat and even some lost bone from the aging process. All of these factors — fine lines and wrinkles, and the loss of fat and bone — cause sinking in, “deflation” which then contributes to sagging skin. “Think of it this way,” Dr. Schultz says, “if you lose some of the fat and some of the bone just by shrinkage, the overlying skin starts to droop because the volume of lost fat and bone was the scaffolding that the skin was hanging on. So if you take away the scaffolding, you’ll get jowling around the face and sagging around jawline and deepening of the lines next to the nose. Filler helps somewhat to restore the scaffolding, but you’ll need tightening to contract it and tighten it up.”
At our office, Dr. Schultz and his team use a different technique than many other dermatology practices. When we first started skin tightening many years ago, the suggestion was to do it every three weeks. While most practices spread the treatments out over months, we realized if we did it more frequently we could achieve better results because more frequent treatments create a great additive effect. Combining this with the fact that we will always give you a customized plan based on your desired results means that you’re always getting the best option for tightening and firming your sagging skin. Dr. Schultz is always testing new technology and only uses the devices that produce the best results so you can be sure you’ll always have the best tools available. You can choose from a lot of different medical devices to have your skin tightened. You may be looking at Ultherapy and Thermage and those can give you good results, but not nearly as consistently as the results you’ll get with Exilis and IPL at our office. As Dr. Schultz says, “It's not just the machine, it's the operator! We get superior results based on experience and technique.”
When you come in for your appointment, you will discuss your desired results with our team. We’ll decide on the best combination or course of treatment for your skin care goals and begin your appointment. If we choose the IPL medical device, there will only be a slight warm feeling to the skin while it’s being administered. For Exilis, there will be more heat but it’s quick and over immediately. There may be a little bit of redness from Exilis but it will disappear in twenty minutes. Once you’re finished with your appointment, whether you’ve used IPL or Exilis, there is no downtime and you can go right back to life!
